Community Impact Through Innovation Award


Celebrating Health & Human Service Organizations Using Technology To Drive Positive Change In Their Communities

Qualifacts and OPEN MINDS are proud to recognize health and human service organizations that are championing a human-centered approach to technology, ensuring digital tools and innovations enhance community health. This award honors organizations leveraging technology to deliver client-centered care, support clinician well-being, and strengthen collaboration across care teams, ultimately improving health outcomes at the community level.

The award will be presented on stage during the opening keynote at The 2026 OPEN MINDS Service Excellence Institute on Wednesday, August 12, 2026, at 8:15 a.m. PST.

Applicants will be scored based on the following criteria:

Innovation: The creativity and originality in using technology to improve individual and/or community health

Impact: Demonstrated outcomes and performance data showing measurable improvements in individual and/or community health

Scalability: The potential for broader adoption across the field, including implementation feasibility, reimbursement or return on investment (ROI), and consumer/community uptake.

One organization will be selected as the award recipient and will receive:

  • Complimentary registration for their team to The 2026 OPEN MINDS Service Excellence Institute
  • National recognition during the opening keynote on August 12
  • Recognition in a press release and social media announcements
  • A written case study of the innovation, inclusion in select award promotions, and the opportunity to present an industry webinar hosted by Qualifacts
  • Feature in an executive briefing on the OPEN MINDS platform, reaching 60,000+ executives
  • An opportunity to present the innovation during a future OPEN MINDS Institute, with a recording hosted in the OPEN MINDS Executive Education Library

Please discuss improvements in access to care, clinical outcomes, system performance, cost reductions, and return on investment for consumers, provider organizations, payers, and/or communities. Include any relevant data related to consumer and customer satisfaction or feedback.
