Community Impact Through Innovation Award


Recognizing Qualifacts Partner Organizations Advancing Technology-Enabled Care

Qualifacts is proud to recognize partner organizations that are leveraging advanced technology, AI-enabled solutions, interoperability, data strategy, and digital innovation to improve care delivery, operational performance, workforce effectiveness, and community outcomes across the health and human service field.

This award recognizes Qualifacts partner organizations that are using technology in meaningful, measurable ways to improve access to care, strengthen consumer engagement, support clinician effectiveness, enhance operational decision making, and drive positive community impact.

The award will be presented on stage during the opening keynote at The 2026 OPEN MINDS Service Excellence Institute on Wednesday, August 12, 2026, at 8:15 a.m. PST.

Applications will be evaluated based on the following criteria:

  • Innovation
    The creativity and strategic use of technology, AI, interoperability, analytics, automation, or digital tools to improve care delivery, organizational performance, workforce effectiveness, and/or community health outcomes.
  • Impact
    Demonstrated measurable outcomes related to access to care, consumer outcomes, operational performance, workforce impact, financial sustainability, quality improvement, and/or community impact.
  • Scalability
    The potential for broader implementation and long-term sustainability across the field, including operational feasibility, reimbursement or ROI potential, workforce adoption, and consumer/community engagement.

One Qualifacts partner organization will be selected as the 2026 award recipient and will receive:

  1. Complimentary registration for their team to The 2026 OPEN MINDS Service Excellence Institute
  2. National recognition during the opening keynote on August 12
  3. Recognition in press releases, digital marketing campaigns, and social media promotions
  4. Development of a written case study highlighting the organization’s innovation and outcomes
  5. The opportunity to participate in a Qualifacts-hosted industry webinar
  6. Featured recognition through the OPEN MINDS platform, reaching more than 60,000 health and human service executives
  7. An opportunity to present the innovation during a future OPEN MINDS institute, with session recording hosted within the OPEN MINDS Executive Education Library\
