

Celebrating Health & Human Service Organizations Using Technology To Drive Positive Change In Their Communities
Qualifacts and OPEN MINDS are proud to recognize health and human service organizations that are championing a human-centered approach to technology—ensuring that digital tools and innovations are truly enhancing community health. This award honors organizations that are leveraging technology to deliver client-centered care, support clinician well-being, and strengthen collaboration across care teams—ultimately improving health outcomes at the community level.
The application is open to health and human service provider organizations until June 27, 2025. The award will be presented on stage at the opening of The 2025 OPEN MINDS Service Excellence Institute’s opening keynote on Wednesday, August 13, 2025 at 8:15 a.m. PST. The award will be presented by Josh Schoeller, Chief Executive Officer of Qualifacts, and Monica E Oss, Chief Executive Officer at OPEN MINDS.
Applicants will be scored based on the following criteria:
- Innovation – The creativity and originality in using technology to improve individual and/or community health
- Impact – Demonstrated outcomes and performance data showing measurable improvements in individual and/or community health
- Scalability – The potential for broader adoption across the field, including implementation feasibility, reimbursement or return on investment (ROI) potential, and consumer/community uptake
One organization will be selected as the award recipient and will receive:
- Complimentary registration for their entire team to The 2025 OPEN MINDS Service Excellence Institute
- National recognition of your organization and innovation as the award recipient during the opening keynote on August 13th
- Recognition of your organization and innovation in a press release and social media announcements
- Receive a written case study of your innovation with inclusion in select award promotions and the opportunity to present as an industry webinar, hosted by Qualifacts
- Featured in an executive briefing on the OPEN MINDS platform, read by 60,000+ executives
- An opportunity to present your innovation during a session at a future OPEN MINDS Institute; a recording of this session will be hosted on OPEN MINDS Executive Education Library
Who Can Apply?
- U.S. provider organizations serving consumers with behavioral health (mental health, substance use disorders) conditions and/or cognitive disabilities (I/DD, autism, dementias, traumatic brain injuries)
- Organizations that have deployed a technology to improve consumer care – with a better consumer experience and/or better outcomes – that has had a meaningful impact on their community
